What is the AMFC program?

Get paid by Florida Medicaid to care for your child at home

The Aide for Medically Fragile Children (AMFC) program was established by Florida House Bill 391 (2023) and allows qualified parents, legal guardians, and caretaker relatives to become certified, paid home health aides for their own medically fragile child.

SabalCare provides the complete 76-hour training program at no cost to your family — and handles all the paperwork, employment setup, and Medicaid authorization from start to finish.

You may qualify if:

  • Your child is under 21 and enrolled in Florida Medicaid
  • Your child receives or qualifies for Private Duty Nursing (PDN) services
  • You are the child's parent, legal guardian, or caretaker relative
  • You are 18 years of age or older
  • You are willing to complete SabalCare's free 76-hour certification program

How to get started

Seven steps from your first call to your first paid shift

SabalCare guides you through every step. You focus on your child — we handle the rest.

Call SabalCare

Reach out to our enrollment team. We'll confirm your child qualifies and explain every step in plain language — in English or Spanish.

Bring your documents

You'll need proof of your relationship to your child (birth certificate or guardianship papers) and a government-issued photo ID confirming you're 18 or older. If you've completed prior home health aide training, bring that too — it may count toward your certification.

We'll give you a complete list before your first visit.

SabalCare handles the DCF paperwork

We complete the required attestation with the Department of Children and Families on your behalf and obtain the necessary case number. You don't need to contact DCF directly.

Sign your employment agreement

You become a SabalCare employee — this is what allows Medicaid to pay you. Your pay rate and schedule are negotiated with SabalCare before training begins.

Complete the 76-hour training program

SabalCare's training is built around your child's specific plan of care. Phase 1 covers core home health aide foundations. Phase 2 focuses on the skills your child actually needs. Phase 3 is hands-on validation with your child — all at no cost to you.

Pass your competency validation

A SabalCare registered nurse observes you performing each required care skill during a real visit with your child. You must demonstrate 100% proficiency. If any skill needs more practice, we work with you before re-validation.

Start getting paid

Once SabalCare receives Medicaid plan authorization, you can begin providing paid care. You'll use our Axxess Home Care app to check in and out of every visit. Your certification renews annually — SabalCare manages the process for you.

The training program

76 hours of free training, built around your child

Unlike a one-size-fits-all program, Phases 2 and 3 are based entirely on your child's individualized plan of care. You only learn and get tested on what your child actually needs.

Phase 1
40hrs

Home health aide training

Foundational skills every caregiver needs — vital signs, personal care, infection control, emergency response, documentation, and more. Taught by a qualified RN.

Phase 2
20hrs

Skills specific to your child

Hands-on training in the skills your child's plan of care requires — tracheostomy care, enteral feeding, seizure management, prescribed equipment, and more. In-person with a pediatric RN.

Phase 3
16hrs

Clinical validation with your child

Your SabalCare RN observes you providing each required skill during real care visits with your child. 100% proficiency required. Conducted on-site.

Training is personalized. You will only be trained on and validated in the specific skills your child's plan of care requires. If your child doesn't use a ventilator, you won't be tested on ventilator care.
